The recently reported statistics as to the alarmingly high death rates at Hull & East Yorkshire Hospitals NHS Trust make for worrying reading. For all those families personally affected it must be incredibly difficult coming to terms with an avoidable death. |

I act for Karen Bonehill, who appeared on
BBC's The One Show, and therefore have personal knowledge of such a situation; our investigations revealed that her husband’s death was entirely preventable and should not have occurred.
Due to a combination of systemic and individual errors, Mr Bonehill was not called back for a further ultrasound scan of his lower leg 1 week after his initial attendance at the A&E Department, Hull Royal Infirmary. As a result, he was denied the straightforward medical treatment that would have saved his life. Legal proceedings are ongoing to make sure that Mr Bonehill’s family receive appropriate compensation.
My colleague, Tim Slow, acted for the parents of Ava Sims Teskey-King, who were also interviewed on The One Show. Again because of simple avoidable mistakes this resulted in Ava being born severely brain damaged and her tragic early death at only 21 months of age.
It is impossible to be unmoved by the sad circumstances of this particular case. Our investigations helped her parents discover what treatment both Ava, and her mother, should have received at and around the time of her birth and the serious errors that occurred.
